Также игрок должен следить за здоровьем Карла, регулярно питаясь в фаст-фуд закусочных (например, Cluckin Bell), в столовых, барах и ночных клубах, перекусывая в автоматах, или покупать еду у продавцов на улице. Поглощение пищи и физические упражнения влияют на мускулатуру и внешность главного героя. Если Карл съест слишком много за короткий промежуток времени, то почувствует недомогание и его стошнит, а если он переедает регулярно, то растолстеет и станет медленнее ходить.

Игроки могут взойти на 800-метровую гору Чилиад (в реальности — гора Дьябло), спрыгнуть на парашюте с различных возвышенностей и небоскрёбов и посетить 12 городков и деревень в трёх округах: Округ Ред (англ. Red County), Округ Флинт (англ. Flint County) и Округ Боун (англ. Bone County), крупную дамбу с гидроэлектростанцией (основана на плотине Гувера, расположенной на границе штатов Аризона и Невада), огромную секретную военную базу под названием «Area 69» (явный намёк на известную «Area 51»), радарную установку и множество других географических достопримечательностей. Бытовали и менее известные мифы (о наличии в игре Лох-Несского чудовища, некого «серийного убийцы», призрака матери Карла Джонсона, а также о загадках, связанных с инвалидной коляской на пирсе озера, в деревне Паломино Крик), также не получившие никаких достоверных подтверждений. Однако все доказательства оказались поддельными.
Во время миссии Black Project в Зоне 69 в одной из лабораторий можно заметить на столе монтировку — явный намек на Half-Life. «Вступление» (The Introduction) — видеоролик, созданный на движке игры, который вышел на DVD вместе с отдельно изданным саундтреком GTA: San Andreas.
