За бой кулаками и кулачным оружием отвечает навык «Без оружия», за бой холодным — «Холодное оружие», за бой огнестрельным малого веса и размера (пистолеты, автоматы, ружья, винтовки и другое) — «Лёгкое оружие», за бой большого веса и размера (гранатомёты, миниганы, огнемёты и другое) — «Тяжёлое оружие», за бой лазерного и плазменного — «Энергооружие», за взрывчатку (мины и гранаты) — «Взрывчатка». Имеется возможность обезвреживать поставленные мины, подкладывать взрывчатку в инвентарь персонажей (с последующим взрывом).
В 16 лет персонаж проходит традиционный тест «К. А.», с помощью которого определяется будущая профессия жителей убежища. В 19 лет протагонист узнаёт, что его отец ушёл из Убежища. Сын или дочь решает последовать за ним и покидает Убежище.
Традиционная фраза «Война, война никогда не меняется.» продолжала использоваться. Для повышения здоровья также может использоваться обычная вода, которая в игре делится на радиоактивную и очищенную. Постоянные спутники — это собака по кличке Псина и ещё один компаньон, которым может быть человек, гуль, супермутант или робот. Саундтрек игры состоит из хитов американской музыки 40-х и 50-х годов XX века, с большим количеством песен, которые ознаменовали эпоху таких исполнителей, как Коул Портер и Билли Холидей. Система опыта под названием S.P.E.C.I.A.L.
После этого игра была допущена к продаже в Австралии. В отчёте комиссии можно прочитать, что наркотики не были полностью удалены из игры, однако были убраны анимации, показывающие их применение. В интервью для британского журнала Edge Bethesda показала, что по всему миру появится только одна версия игры, в которой названия наркотиков, существующие в реальном мире, будут удалены. Позже выяснилось, что единственное изменение коснулось морфина, который был переименован в «Med-X».
Fallout 3 (англ.) на сайте IGN Руководитель — президент Джон Генри Эдем. Оставшиеся файлы я удалил с помощью Auslogicks BoodSpeed Premium. При уменьшении количества очков действия данный показатель произвольно восстанавливается с течением времени.
